New Patients are always welcome at Hudson Chiropractic. Click on the link to the right for downloadable forms you can bring with you on your first visit.
Please bring in your insurance card at your first visit. Worker's Compensation and No-fault clients must bring in all information about their claims on the first visit.
If you have recent x-rays, MRIs or other imaging studies from previous doctors, please bring those with you as well.
